Creates the Veterans' Prosthesis Fund as a special fund in the State treasury to be administered by the Department of Veterans' Affairs for grants to Illinois veterans to update and replace prostheses.

Establishes a disabled veterans income tax checkoff on Illinois state income tax forms for taxable years ending on or after December 31, 2012, allowing taxpayers to contribute any amount of $1 or more.

Distributes collected checkoff contributions equally: one-third to the Veterans' Prosthesis Fund, one-third to the Illinois Military Family Relief Fund, and one-third to the Disabled Veterans Property Tax Relief Fund.

Allows the Veterans' Prosthesis Fund to receive moneys from the tax checkoff, public and private donations, and all available federal funds designated for veterans prosthesis assistance.

Takes effect upon becoming law.
