Schools must make immunization data publicly available by December 1 of each year, identical to data reported to the State Board of Education by November 15.

Health examinations required at kindergarten/first grade entry, sixth grade, and ninth grade must include obesity data collection (date of birth, gender, height, weight, blood pressure, exam date) and diabetes screening.

Dental examinations required for kindergarten, second grade, and sixth grade students with proof due by May 15; schools may withhold report cards until proof is provided or appointment scheduled within 60 days.

Eye examinations required for kindergarten entrants and first-time school enrollees, with proof due by October 15; schools cannot exclude children for lack of examination but may withhold report cards.

Parents may obtain religious or medical exemptions from health, dental, and eye examinations and immunizations by submitting signed statements of objection to local school authorities.
