Amends the Illinois Income Tax Act to require the Department of Revenue to print a provision on standard individual income tax forms allowing taxpayers to voluntarily contribute to the General Revenue Fund starting with taxable years ending on or after December 31, 2012

Taxpayers may contribute any amount of at least $1 by stating the contribution amount on their return, which will reduce their refund or increase their payment amount owed

If a taxpayer fails to remit the full increased payment amount, the contribution is reduced accordingly, and this provision does not apply to amended returns

The provision is not classified as an income tax checkoff under Section 509.1 of the Act and must remain on the standard form regardless of contribution amounts received in prior years

Takes effect immediately upon becoming law
