Exempts the Diabetes Research Checkoff Fund from the $100,000 minimum contribution threshold required to remain on individual income tax return forms.

Ensures the Diabetes Research Checkoff Fund is included on tax return forms for taxable years ending on or after December 31, 2012.

Excludes the Diabetes Research Checkoff Fund from the 15 tax-checkoff fund limitation imposed on the individual tax return form.

Allows individual income tax return forms to contain explanations and spaces enabling taxpayers to designate contributions to authorized checkoff funds.
