Authorizes the Secretary of State to issue "Choose Life" special license plates to demonstrate support for organizations promoting adoption as an alternative to abortion.

Charges applicants a $25 fee at original issuance and $25 at renewal in addition to regular registration fees, with $10 and $23 respectively deposited into the Choose Life Fund.

Creates the Choose Life Fund as a special state treasury fund to distribute moneys on a pro-rated basis to qualified non-profit agencies providing counseling and material support to pregnant women planning adoption.

Restricts funding to organizations that do not provide abortion services, counseling, or referrals, do not charge women for services, and allocate no more than 60% of funds to pregnant women's material needs and no more than 40% to adoption promotion efforts.

Requires recipient organizations to submit annual notarized affidavits certifying compliance with fund usage restrictions and return any unused moneys exceeding 10% of allocations to the Choose Life Fund.
