Create the Tobacco Dependence Coverage Law requiring group and individual health insurance policies and managed care plans in Illinois to cover up to $500 annually for tobacco use cessation programs for enrollees age 18 and older.

Define tobacco use cessation programs to include physician-recommended evidence-based treatment, counseling by medical personnel, and all FDA-approved medications for tobacco dependence whether available over-the-counter or by prescription.

Prohibit insurers from denying eligibility or renewal coverage solely to avoid these requirements, and ban penalties on healthcare providers or incentives that induce them to provide care inconsistent with the law.

Require written notice to insureds about tobacco cessation coverage availability upon enrollment and annually thereafter.

Exclude short-term travel, disability income, long-term care, accident-only, and limited or specified disease policies from the tobacco cessation coverage requirement.
