Amends the Community Services Act to require the Department of Human Services to submit reports within 6 months and one year after closure of any State-operated mental health or developmental disabilities facility.

Reports must describe the venue of care for former residents, costs to the Department from new care venues, cost savings or increases to former residents, and any injuries or deaths occurring after closure.

Directs net proceeds from sale of closed facility real estate to the Community Mental Health Medicaid Trust Fund (for mental health facilities) or Community Developmental Disability Services Medicaid Trust Fund (for developmental disabilities facilities).

Requires the Division of Mental Health and Division of Developmental Disabilities to allocate funds from sale proceeds to address life, safety, and care concerns at remaining State-operated facilities that will serve individuals previously served at the closed facility.

Mandates the Department of Human Services to share and discuss plans for using sale proceeds with advocates and advocacy organizations whose mission includes advocacy for persons with developmental disabilities or mental illness.
