Amends the Illinois Oil and Gas Act to create new disclosure requirements for hydraulic fracturing operations used to extract hydrocarbons from shale.

Requires well operators to complete and post forms on a public website detailing total water volume used and all chemical ingredients subject to federal Occupational Safety and Health Administration requirements (29 C.F.R. 1910.1200(g)(2)).

Requires service companies and chemical suppliers performing hydraulic fracturing to provide operators with necessary information for compliance and allows entities to claim trade secret protection for chemical identities and amounts under a specified process.

Limits trade secret challenges to landowners on the well site, adjacent landowners, and relevant state agencies, with challenges required within two years of well completion report filing.

Establishes a process allowing health professionals and emergency responders to obtain trade secret information consistent with federal rules, with trade secret protections governed by the Illinois Freedom of Information Act.
