Amends the Community-Integrated Living Arrangements Licensure and Certification Act to establish licensing requirements for community mental health and developmental services agencies operating community-based residential programs.

Sets application and renewal licensing fees at a maximum of $200 each, with licenses valid for 3 years unless suspended, revoked, or voluntarily surrendered.

Prohibits the Department from adopting rules that restrict facility distance requirements and requires adoption of rules promoting community integration and opportunities for additional community-based living arrangements.

Requires the Department to incorporate geographic distribution into selection processes for individuals to be served in community-based living arrangements from the waiting list and PUNS database.

Establishes Department authority to conduct site visits and inspections, issue notices of violation for non-compliance, and initiate license reviews in response to complaints, substantiated abuse cases, unexplained deaths, or other significant events.
