Authorizes the Secretary of State to issue special Alzheimer's Awareness license plates for passenger vehicles of the first division and motor vehicles of the second division weighing not more than 8,000 pounds.

Charges applicants a $25 fee for original issuance and $25 for each renewal period, in addition to standard registration fees.

Allocates $10 from original issuance fees and $23 from renewal fees to the Alzheimer's Awareness Fund; allocates $15 from original issuance fees and $2 from renewal fees to the Secretary of State Special License Plate Fund for administrative costs.

Creates the Alzheimer's Awareness Fund as a special fund in the State treasury to be distributed as grants to the Alzheimer's Disease and Related Disorders Association, Greater Illinois Chapter, for care, support, education, and awareness programs.

Allows plates to be issued as vanity or personalized plates at the Secretary's discretion.
