Urges the Governor to create a Distressed Counties and Communities Commission composed of stakeholders, state agency representatives, and community leaders from distressed areas across Illinois.

Identifies 27 Illinois counties and 7 municipalities with poverty rates of 16% or higher as of 2010, including Cook, Chicago, Rockford, and Springfield, noting these areas face crime, unemployment, inadequate housing, and deficient public services.

Charges the Commission with developing an action plan by April 15, 2013 to improve delivery of state services in law enforcement, corrections, education, job training, economic development, and public health without increasing state taxes.

Requires the Commission to hold public hearings and analyze successful programs from other states and localities that could be adapted to address social and economic problems in distressed communities.

Directs the Governor to designate an executive department to provide administrative support and appoint Commission members by December 31, 2012 to enable the first meeting in January 2013.
