Amends Section 8-2001 of the Code of Civil Procedure to establish procedures for patients and authorized parties to examine and obtain copies of health care records from facilities and practitioners.

Sets specific fees for record copying: $20 handling charge plus 75 cents per page for pages 1-25, 50 cents per page for pages 26-50, and 25 cents per page for pages over 50, with reduced rates for electronic records.

Requires health care facilities and practitioners to fulfill record requests within 30 days, or provide written explanation of delay with a maximum 60-day deadline for compliance.

Modifies Section 8-2001.5 to change deceased patients' record release from permissive ("may") to mandatory ("must"), allowing surviving spouses, adult children, parents, or adult siblings to obtain records with an Authorized Relative Certification and death certificate.

Eliminates the $20 handling fee for patients and personal representatives obtaining deceased patient records under Section 8-2001.5.
