Extends the Quality of Life scratch-off lottery game from December 31, 2012 to December 31, 2017, with net revenue deposited into the Quality of Life Endowment Fund for HIV/AIDS prevention education and treatment grants through the Department of Public Health.

Extends the Quality of Life Board from June 30, 2013 to June 30, 2018, maintaining its advisory role of 11 members from legislative leadership, the Governor, and HIV/AIDS advocacy organizations across Illinois regions.

Creates three new special funds in the State treasury: Childhood Cancer Research Fund (managed by Department of Public Health), Children's Wellness Charities Fund (managed by Department of Human Services), and Housing for Families Fund (managed by Department of Human Services).

Adds three new income tax checkoffs allowing taxpayers to voluntarily contribute at least $1 to the Childhood Cancer Research Fund, Children's Wellness Charities Fund, or Housing for Families Fund, effective for tax years ending December 31, 2012 and later.

Modifies tax checkoff provisions to exclude the Diabetes Research Checkoff Fund and Hunger Relief Fund from the 15-checkoff fund limit on individual income tax return forms.
