Creates Alzheimer's Awareness special license plates available for passenger vehicles of the first division and motor vehicles of the second division weighing not more than 8,000 pounds.

Charges a $25 fee for original plate issuance ($10 to Alzheimer's Awareness Fund, $15 to Secretary of State Special License Plate Fund) plus a $25 renewal fee ($23 to Alzheimer's Awareness Fund, $2 to Secretary of State Special License Plate Fund), in addition to regular registration fees.

Establishes the Alzheimer's Awareness Fund as a special fund in the State treasury to receive revenues from plate fees.

Directs all money in the Alzheimer's Awareness Fund to be distributed as grants to the Alzheimer's Disease and Related Disorders Association, Greater Illinois Chapter, for care, support, education, and awareness programs.

Allows plates to be issued as vanity or personalized plates at the Secretary of State's discretion, with design and color determined by the Secretary.
