Amends the State Budget Law to establish a long-term care rebalancing initiative aimed at removing barriers to community living for people with disabilities and long-term illnesses.

Requires the Governor to create a unified budget report from all state agencies offering long-term care services, including enrollment data, funding sources, and service comparisons broken down by age groups (children under 21, adults 21-64, and seniors 65+).

Mandates each state agency providing long-term care services to analyze progress in transitioning individuals from institutional to community settings.

Authorizes the Governor to transfer up to 4% of long-term care appropriations from institutional service budgets to community-based programs administered by the Department of Healthcare and Family Services, Department of Human Services, and Department on Aging, subject to certification of need.

Requires 30 days' notice to the General Assembly before transferring funds, with posting required on relevant state agency and Governor's office websites.
