Authorizes the Illinois Secretary of State to issue special "Diabetes Awareness" license plates to owners of passenger vehicles and motor vehicles weighing 8,000 pounds or less.

Plates may be issued as vanity or personalized plates at the Secretary of State's discretion, with design and color determined by the Secretary.

Charges a $40 fee for original issuance ($25 to Diabetes Research Checkoff Fund, $15 to Secretary of State Special License Plate Fund for administrative costs).

Charges a $27 renewal fee for each registration period ($25 to Diabetes Research Checkoff Fund, $2 to Secretary of State Special License Plate Fund).

Plates expire according to the multi-year registration procedure established by existing state law.
