Requires nursing homes to install electronic monitoring devices (video cameras and audio devices) throughout facilities in common areas, hallways, dining areas, nurses' stations, and residents' rooms.

Allows residents or their legal representatives to opt out of monitoring in their rooms by executing a waiver witnessed by the Office of State Long Term Care Ombudsman, and permits temporary waivers for privacy during intimate moments.

Establishes recording requirements including motion-activated video in rooms and sound-activated audio in bathrooms, with recordings maintained at the facility for 30 days minimum and transferred to the Department of Public Health for 5-year storage.

Makes intentionally hampering, obstructing, tampering with, or destroying monitoring devices a Class A misdemeanor punishable by a fine up to $5,000.

Allows residents and legal representatives access to real-time and recorded files through secure online access and computer stations at the facility, with recordings available to law enforcement, State's Attorneys, and attorneys upon request.
