Amends the Illinois Act on the Aging to require the Department on Aging to establish a nursing home prescreening program for Alzheimer's Disease victims and disabled or blind individuals needing long-term care, without regard to income guidelines.

Requires case coordination units to conduct prescreening when hospital discharge planners or community providers identify imminent risk of nursing home placement, and to inform individuals of all care options including in-home and community-based services.

Mandates that individuals being prescreened receive information about spousal impoverishment requirements, financial information submission needs, and consequences for non-compliance in a form jointly developed by the Department on Aging, Department of Human Services, and Department of Healthcare and Family Services.

Allows case coordination units under contract with the Department to charge a prescreening fee not to exceed the cost of providing such services.

Requires case coordination units to provide information about the Office of State Long Term Care Ombudsman's Residents Right to Know database at each prescreening.
