IL HB2904

Amends the Illinois Marriage and Dissolution of Marriage Act to allow one or both parties to elect a surname change at the time of marriage by designating it on the marriage license application.

Permits spouses to choose from four options for their post-marriage surname: (1) the other spouse's surname; (2) any former surname of either spouse; (3) a combined single surname using all or part of either spouse's premarriage or former surname; or (4) a hyphenated or space-separated combination surname using only premarriage or former surnames of either spouse.

Requires the Director of Public Health to include a surname designation field on the marriage license application form.

Establishes that the marriage certificate containing the designated surname constitutes proof that use of the new surname or retention of the former surname is lawful, creating an official record of the surname change.

Takes effect immediately upon becoming law.
