Establishes a Sentence Modification Program allowing committed persons aged 50+ who have served 25+ consecutive years to petition the Prisoner Review Board for early release based on demonstrated rehabilitation, remorse, and viable release plans.

Creates a medical parole provision allowing early release for committed persons with terminal or debilitating conditions that render them unlikely to pose danger to society, to hospitals, hospices, or appropriate housing.

Authorizes the Prisoner Review Board to grant participation in the Sentence Modification Program and requires notification of victims and their families who may submit statements supporting or opposing the petitioner's release.

Specifies that the Board must consider factors including successful program participation, genuine reform, remorse, ability to socialize, renunciation of criminal activity, and appropriate living arrangements when deciding on Sentence Modification Program participation.

Allows annual petitions for program participation with a discretionary 3-year waiting period after denial if the Board determines earlier petition would be unreasonable; all persons serving sentences in the Department are eligible to participate.
