Establishes the Voluntary FBI Fingerprint Demonstration Project for long-term care facilities, operating from January 1, 2014 through December 31, 2014 under a Centers for Medicare and Medicaid Services grant.

Requires participating facilities to conduct State and FBI fingerprint-based criminal history record checks on "direct access employees" who have one-on-one contact with residents.

Makes participation voluntary for all long-term care facilities licensed under the Nursing Home Care Act, with the Department of Public Health selecting at least one facility to participate.

Directs the Department of Public Health to notify qualified facilities within 30 days of the effective date and grants facilities 30 days to apply for participation in the demonstration project.

Places responsibility for fingerprint inquiry costs on individual applicants, which may be offset with available grant funds, and allows facilities to terminate participation with 30 days' notice.
