Establishes a cross-disability database maintained by the Department of Human Services for Illinois residents with disabilities potentially needing services, operational by July 1, 2004.

Database includes individuals with mental illness, physical disabilities, developmental disabilities, and autism spectrum disorders, collecting demographic information, service needs, diagnoses, and residential district.

Allows legal residents who are dependents of military service members absent due to military service to be added to the database and retain eligibility for developmental disability services upon return to Illinois.

Dependents of military service members who left the state due to military assignment retain eligibility for developmental disability services as long as they remain legal residents and otherwise qualify, with 6 months to apply and 6 months to commence services if selected.

Prohibits payment for home and community-based services provided outside Illinois, except where federal regulations permit; requires dependents to provide military discharge paperwork (DD-214) and proof of legal state residence.
