Creates Community Mental Health Medicaid Trust Fund and Community Developmental Disability Services Medicaid Trust Fund to receive 100% of federal Medicaid reimbursements for community mental health and developmental disability services.

Requires net proceeds from the sale of closed State mental health or developmental disabilities facilities to be deposited into the respective trust funds, with up to 25% of proceeds available for infrastructure needs at remaining facilities.

Mandates that at least 40% of mental health facility sale proceeds be spent in the facility's original geographical area, with an additional 20% for nearby facilities closed within one year; establishes specific approved uses including permanent supportive housing, crisis services, and employment support.

Establishes the Human Services Priority Capital Program Fund to provide long-term below-market loans to nonprofit human service providers for capital needs, with facility sale proceeds eligible for deposit into this fund.

Requires annual reports to the Governor and General Assembly by September 1 detailing revenues and expenditures from both trust funds, to be published on the Department of Human Services website at least one week before presentation to the legislature.
