IL SB1550

Defines "transition services" for students with disabilities as coordinated activities designed to improve academic and functional achievement and facilitate movement from school to post-secondary education, employment, independent living, or community participation.

Requires individualized education plans (IEPs) beginning no later than when a student turns age 14½ to include measurable post-secondary goals related to training, education, employment, and independent living skills, along with necessary transition services.

Mandates that transition planning include identification of persons responsible for coordinating services, and if outside agencies are needed, requires IEP teams to identify resources, make referrals, and monitor student progress toward transition goals with specific review periods.

Allows additional transition planning participants including representatives from the Department of Human Services, state agencies, case coordinators, and public or community service providers to participate in IEP development.

Requires school districts to submit annual summaries of eligible students' IEP transition goals to local Transition Planning Committees and provide transition assistance to students with disabilities who are ineligible for special education services.
