Amends Section 509 of the Illinois Income Tax Act regarding tax checkoff explanations on individual income tax return forms.

Makes a technical grammatical change by replacing "and and" with "and" in the section describing explanations and spaces for taxpayer contributions.

Requires the Department to include the Hunger Relief Fund checkoff on individual income tax forms for the taxable year beginning January 1, 2012.

Mandates removal of checkoff explanations from tax forms if total contributions to any fund do not reach $100,000 by October 1 of any year, with subsequent contributions refunded to taxpayers.

Exempts the Diabetes Research Checkoff Fund from the $100,000 contribution threshold requirement.
