Authorizes the Secretary of State to issue special Alzheimer's Awareness license plates for passenger vehicles and motor vehicles weighing 8,000 pounds or less.

Charges applicants a $25 fee for original issuance and $25 at each renewal, in addition to regular registration fees.

Allocates $10 of the original issuance fee and $23 of the renewal fee to the Alzheimer's Awareness Fund; allocates $15 of the original issuance fee and $2 of the renewal fee to the Secretary of State Special License Plate Fund.

Creates the Alzheimer's Awareness Fund as a special fund in the State treasury to distribute grants to the Alzheimer's Disease and Related Disorders Association, Greater Illinois Chapter, for care, support, education, and awareness programs.

Permits plates to be issued as vanity or personalized plates under existing vehicle code provisions.
