Establishes a Health Homes for Chronic Conditions Pilot Program under the Illinois Medical Assistance program, utilizing licensed or certified developmental disability and behavioral health service providers as designated providers.

Health homes must provide minimum services including comprehensive care management, care coordination, health promotion, comprehensive transitional care/follow-up, patient and family support, and referral to community and social support services, consistent with Section 2703 of the Patient Protection and Affordable Care Act.

Requires the Department of Healthcare and Family Services to submit a State Plan amendment application to the federal Centers for Medicare and Medicaid Services by January 1, 2015 to implement the pilot program.

Aligns with Illinois Public Act 96-1501's long-term care rebalancing initiative and the State's Alliance for Health Innovation Plan to improve person-centered care coordination for individuals with intellectual and developmental disabilities, mental illness, or substance use disorders.

Takes effect immediately upon becoming law.
