Department of Public Health must develop and publish a uniform "DNR/POLST form" (Practitioner Orders for Life-Sustaining Treatment) in consultation with physician, nursing, hospital, and emergency medical services organizations, with publication required by January 1, 2015.

Department of Public Health shall publish Spanish language versions of advance directive forms including Living Will Declaration, Power of Attorney for Health Care, Declaration of Mental Health Treatment Form, advance directives law summary, and the DNR/POLST form.

Nursing homes, hospitals, ID/DD community care facilities, and EMS services must establish policies honoring practitioner orders for CPR and life-sustaining treatment, including the Department's uniform DNR/POLST form and previous versions of uniform forms.

Expands authority to execute DNR/POLST forms from physicians only to any attending health care practitioner (including advanced practice nurses, physician assistants, and licensed residents after one year of training), and makes execution voluntary with annual review recommended.

Health care professionals and providers acting in good faith to comply with DNR/POLST forms are protected from criminal, civil, and professional conduct liability except for willful and wanton misconduct.
