Creates new Section 355.4 of the Illinois Insurance Code prohibiting insurers from requiring optometrists or ophthalmologists to provide services or materials at insurer-set fees unless those services or materials are covered under the policyholder's plan agreement.

Defines "covered services" as vision care services for which reimbursement is available under an enrollee's plan, including services that would be covered but for contractual limitations like deductibles, copayments, and coinsurance.

Specifies that de minimis (minimal) reimbursements do not qualify a service or material as a covered service or material.

Applies the noncovered vision services restrictions to health maintenance organizations, limited health service organizations, and voluntary health services plans by amending their respective governing statutes.

Effective for insurance policies issued, delivered, amended, or renewed on or after the bill's effective date.
