Amends the Illinois Public Aid Code to extend the 4-year moratorium on medical assistance program eligibility expansions if the Department of Healthcare and Family Services fails to achieve 50% care coordination enrollment by January 1, 2015.

The moratorium extension shall last an additional 2 years or until the 50% care coordination enrollment mandate is met, whichever is longer.

Adds a concurrent moratorium on benefit expansions to run alongside the existing moratorium on eligibility expansions during any extended period.

The original moratorium prohibits expanding eligibility through increasing financial standards, income disregards, or creating new eligible categories beyond those covered on January 1, 2011.

Takes effect immediately upon becoming law.
