Establishes a 3-year pilot program requiring electrocardiogram (EKG) tests as part of health examinations for student athletes in Chicago Public Schools (District 299), with parents responsible for EKG test costs.

Requires the Department of Public Health and State Board of Education to develop and post online guidelines about sudden cardiac arrest symptoms and warning signs for students, parents, and coaches, including fainting, chest pain, difficulty breathing, dizziness, and abnormal heart rate.

Mandates that students exhibiting sudden cardiac arrest symptoms during athletic activities be immediately removed from participation and cannot return until cleared in writing by a physician, certified nurse practitioner, or cardiologist.

Requires coaches to complete annual sudden cardiac arrest training from Department-approved providers and establishes escalating penalties for violations: suspension for remainder of season (first violation), suspension for remainder and next season (second violation), and permanent suspension (third violation).

Requires students and parents to sign acknowledgment of receipt of a sudden cardiac arrest information sheet each school year prior to athletic participation.
