Creates a voluntary income tax checkoff allowing Illinois taxpayers to contribute to the U.S.S. Illinois Commissioning Fund for the 2014-2016 tax years.

Department of Revenue must print the checkoff provision on standard individual income tax forms, allowing taxpayers to contribute any amount of at least $1.

Contributions reduce the taxpayer's refund or increase the payment amount owed with the return, and any shortfall in payment reduces the contribution accordingly.

Department of Military Affairs distributes collected funds as grants to the U.S.S. Illinois Commissioning Committee.

Both the checkoff provision and the U.S.S. Illinois Commissioning Fund are repealed effective December 31, 2017.
