Establishes "Choose Life" special license plates in Illinois that demonstrate support for adoption as an option for women with unplanned or unwanted pregnancies.

Charges a $25 fee for original issuance and $25 for renewal; $10 and $23 respectively go to the new Choose Life Fund, with remainder to Secretary of State Special License Plate Fund for administrative costs.

Creates the Choose Life Fund as a special state treasury fund, with moneys distributed pro-rata by county to qualified non-profit organizations that provide counseling and material support to pregnant women planning adoption.

Restricts Choose Life Fund distributions to organizations that do not perform or promote abortion services, do not charge women for services, and agree to use no more than 60% of funds for material needs and no more than 40% for adoption promotion activities.

Requires organizations to submit annual notarized affidavits certifying compliance with fund usage restrictions, and mandates return of any unused funds exceeding 10% to the Choose Life Fund for redistribution.
