Creates the Mental Health Income Tax Checkoff Fund as a special fund in the state treasury, with moneys to be used by the Department of Public Health for grants to mental health service providers.

Requires the Department to print a checkoff provision on standard individual income tax forms for tax years ending on or after December 31, 2015, allowing taxpayers to contribute a minimum of $1 to the Mental Health Income Tax Checkoff Fund.

Specifies that taxpayer contributions will reduce their refund or increase their payment due, and failure to remit any increased payment will reduce the contribution accordingly.

Clarifies that the checkoff provision does not apply to amended tax returns.

Takes effect immediately upon becoming law.
