Amends Illinois School Code Section 27-8.1 to add mental health examination as a required component of health examinations for all students.

Requires health examinations to include collection of obesity-related data (date of birth, gender, height, weight, blood pressure, and exam date) and diabetes screening as defined by Department of Public Health rules.

Mandates dental examinations for students in kindergarten and 2nd and 6th grades, with proof required by May 15th each school year; schools may withhold report cards until proof is provided or examination is scheduled within 60 days.

Requires eye examinations for kindergarteners and students enrolling for the first time, with proof needed by October 15th; Department of Public Health must establish waivers for undue burden or lack of access to eye care providers.

Establishes reporting requirements to the State Board of Education on immunization, health examination, dental examination, and eye examination compliance by specified deadlines, with data made publicly available by December 1st annually.
