Adds requirement that care coordination services for Medicare-Medicaid Alignment Initiative Demonstration Project enrollees protect all rights guaranteed under federal and state law, including the right to select one's own physician.

Requires managed care organizations to change an enrollee's primary care provider within 24 hours of receiving a request and notify the enrollee in writing.

Mandates that if an enrollee declares a medical emergency exists, the managed care organization shall make primary care provider changes immediately, notify all involved parties by phone, and follow up with written confirmation.

Preserves existing continuity of care requirements for nursing home residents transitioning to managed care organizations, including honoring existing care plans until comprehensive assessments are completed.
