Creates the Health Care Cost Estimate Act requiring health care providers to disclose the allowed amount or charge for a proposed admission, procedure, or service within 2 working days upon patient request.

Requires health care providers to disclose estimated maximum allowed amounts when unable to predict specific treatment or diagnostic codes in advance.

Mandates health insurance companies to establish toll-free telephone numbers and Internet websites enabling consumers to obtain in real time the estimated or maximum allowed amounts for proposed services and the estimated out-of-pocket costs for covered benefits.

Requires network health care providers to furnish patients with sufficient information to use the insurance company's toll-free number and website to obtain cost estimates, and allows providers to assist patients in accessing these tools.

Applies to all companies issuing, delivering, amending, or renewing individual or group accident and health insurance policies in Illinois.
