Expands the Illinois Oil and Gas Act to prohibit falsification or misstatement of information on applications, permits, required records, or documents submitted to the Department.

Increases civil penalties for falsifications or misstatements to $5,000 per violation, and maintains $1,000 per day penalties for other violations.

Authorizes the Department Director to affirm, vacate, or modify notices of violation after investigation, with consideration of violation history, seriousness, and culpability.

Allows the Director to impose remedial actions, civil penalties, permit modifications with special monitoring requirements, or permit revocation.

Provides that applicants or permit holders with officers, directors, or partners holding more than 5% interest in entities that failed to abate violations may be denied permits or have permits revoked.
