Creates a Person with a Disability Wallet Card available to individuals approved for Illinois Person with a Disability Identification Cards with Type Two or Type Five disabilities.

Secretary of State must inform applicants of the wallet card's availability and provide the card upon request.

Card specifies that the cardholder has been medically diagnosed with a disability to help inform criminal justice professionals during interactions.

Department of Human Services designs and produces the wallet cards in consultation with the Secretary of State, then distributes them for issuance.

Secretary of State and Department of Human Services must adopt rules for administering the wallet card program.
