Department of Financial and Professional Regulation shall issue identification cards to massage therapists and nail technicians beginning January 1, 2018, containing name, license number, residence, date of birth, sex, physical description, photograph, signature, and license expiration date.

Department shall use digital photograph and signature from Illinois driver's license or Illinois Identification Card when available; Secretary of State must provide access to these materials for identification purposes.

Licensees age 21 or older may request religious exemption from photograph requirement by providing IRS Form 4029; those granted exemptions must submit fingerprints instead.

Forging, materially altering, counterfeiting, or knowingly possessing forged or counterfeit identification cards constitutes a Class 2 felony; aiding or abetting such conduct also constitutes a Class 2 felony.

Massage therapists and nail technicians must carry and display their identification cards in a conspicuous place at their principal office or work location to legally practice their profession.
