Requires individual and group health insurance policies amended, delivered, issued, or renewed in Illinois after the effective date to cover all FDA-approved contraceptive drugs, devices, and products without cost-sharing, excluding male condoms.

Mandates coverage for voluntary sterilization procedures, contraceptive services, patient education, counseling, and follow-up services including device insertion and removal.

Allows insurance plans to include only one therapeutic equivalent version of a contraceptive drug or device on their formulary if multiple FDA-approved versions exist, provided at least one is covered without cost-sharing.

Requires coverage of 12 months' worth of contraception dispensed at one time and establishes an accessible appeals process for non-covered contraceptives based on attending provider medical necessity determinations.

Becomes inoperative if federal regulations require the State to defray costs under the Affordable Care Act; in that case, a fallback provision applies coverage requirements only to the extent authorized by Social Security Act Section 1902.
