Creates the Prostate Cancer Awareness Fund as a special fund in the State treasury to receive fees from Universal special license plates issued by the Prostate Cancer Foundation of Chicago.

Authorizes the Prostate Cancer Foundation of Chicago to issue decals for Universal special license plates with an original issuance fee of $25 ($10 to the Prostate Cancer Awareness Fund and $15 to the Secretary of State Special License Plate Fund).

Sets renewal fee at $25 with $23 going to the Prostate Cancer Awareness Fund and $2 to the Secretary of State Special License Plate Fund.

Allows Universal special license plates to be affixed only to passenger vehicles of the first division (including motorcycles and autocycles) or motor vehicles of the second division weighing not more than 8,000 pounds.

Requires charitable entities receiving plate fees to annually provide the Secretary of State with a letter of compliance from the Attorney General verifying compliance with the Charitable Trust Act and Solicitation for Charity Act.
