Amends the Vital Records Act to change terminology in paternity and parentage acknowledgment forms from "acknowledgment of parentage" to "voluntary acknowledgment of paternity" and "denial of paternity" to "denial of parentage."

Requires the voluntary acknowledgment of paternity and denial of parentage forms to contain data elements required by federal law rather than specific formatting requirements (bold-face capital letters of specified height).

Requires the Department of Healthcare and Family Services to provide instructions to hospital personnel about new data elements added to voluntary acknowledgment of paternity, denial of parentage, and rescission forms in coordination with the Department of Public Health.

Establishes that signing and witnessing of voluntary acknowledgment of paternity or denial of parentage forms conclusively establish a parent-child relationship in accordance with the Illinois Parentage Act of 1984 or Illinois Parentage Act of 2015.

Allows parties to rescind voluntary acknowledgment of paternity or denial of parentage forms within specified timeframes, with rescission voiding the acknowledgment and nullifying the presumption of paternity.
