Requires physicians to report to the Illinois Department of Public Health within a reasonable time period when a woman is diagnosed with complications from an abortion or dies as a result of an abortion.

Directs the Department of Public Health to define reportable complications by rule, using contemporary medical standards with severity equal to or greater than hemorrhaging requiring transfusion, infection, incomplete abortion, or punctured organs.

Mandates the Department of Public Health to compile and annually publish data on abortion-related deaths or complications when more than 5 cases occur in a county or statewide, broken down by state and county.

Prohibits public data from including facility names, patient identification numbers, datasets with 50 or fewer cases that could identify individuals, education level, or race and ethnicity of patients.

Subjects physicians who intentionally violate this reporting requirement to license revocation under the Medical Practice Act of 1987.
