Urges the Illinois Department of Commerce and Economic Opportunity to create and distribute a survey to identify the main reasons Illinois residents choose to leave the state.

Requests the Department make reasonable efforts to have former Illinois residents complete the survey to understand their decision to relocate.

Directs the Department to utilize cost-free online survey websites to minimize expenses for creating the survey.

Requires the Department to provide survey results to the General Assembly by December 1, 2016.

Addresses Illinois's recent negative population growth, with approximately 94,000 residents relocating in 2014.
