Directs the Department of Public Health to conduct a study in coordination with the University of Illinois at Chicago College of Pharmacy, Southern Illinois University Edwardsville School of Pharmacy, and Chicago State University College of Pharmacy on improving communication between healthcare providers regarding patient medication changes.

Urges the Department of Public Health to examine and recommend solutions for electronically transmitting "discontinuation," "cancel," or "stop" notifications to pharmacies when prescribers discontinue or cancel electronically-prescribed prescription orders.

Requests the Department of Public Health to evaluate the benefits of mandated pharmacist-led medication reconciliation at patient entry into new healthcare settings, discharge education, follow-up communication, and electronic notification to pharmacies of medication changes.

Proposes using the primary care provider as a central nexus for communication between all healthcare providers and pharmacists to maintain a centralized medication list for each patient.

Requires the Department of Public Health to submit its findings and recommendations to the General Assembly, Governor, and Secretary of Public Health by January 1, 2017.
