Amends the Illinois Insurance Code to require insurers to cover low-dose mammography screening for women 35 and older, including baseline mammograms for ages 35-39, annual mammograms for age 40+, and mammograms as medically necessary for younger women with risk factors.

Expands the definition of "low-dose mammography" to include breast tomosynthesis, defined as a radiologic procedure producing cross-sectional three-dimensional images of the breast.

Requires coverage of mammography screening at no cost to the insured and prohibits application to annual or lifetime maximum benefits.

Mandates coverage for breast reconstruction, prosthetic devices, and treatment for complications following mastectomy, including lymphedema treatment, with care determined in consultation with the physician and patient.

Applies identical mammography and mastectomy coverage requirements to Health Maintenance Organizations and the Illinois Medical Assistance (Medicaid) program.
