Requires insurance policies (individual and group accident and health insurance, managed care plans, HMOs, and voluntary health services plans) amended, delivered, issued, or renewed after the bill's effective date to cover hearing instruments and related services when prescribed by a licensed audiologist or physician.

Mandates coverage of up to $2,500 per hearing aid per insured's hearing impaired ear, including audiological exams, replacement ear molds, and repairs, subject to applicable co-payments, co-insurance, deductibles, and out-of-pocket limits.

Limits hearing aid replacement to once every 36 months, except when a significant change in hearing (10 decibels or greater on audiometric testing) occurs, which allows earlier replacement regardless of the 36-month restriction.

Prevents insurers from covering a new hearing aid claim if the same insured filed a claim for a hearing aid within the previous 36 months that was paid by any insurer.

Applies coverage requirements to the Illinois Insurance Code, Health Maintenance Organization Act, and Voluntary Health Services Plans Act; effective immediately upon passage.
