Creates the Supplemental Low-Income Energy Assistance Fund to receive voluntary donations and utility customer surcharges, with funds used for utility bill assistance, weatherization services, and program administration.

Requires all public utilities, electric cooperatives, and municipal utilities in Illinois to assess monthly Energy Assistance Charges on customer accounts: $0.48 per month for residential electric and gas service, and $4.80-$360 per month for non-residential accounts based on usage levels.

Limits yearly weatherization expenditures to 10% of collected charges and administrative expenses to 10% of collected charges, except unspent administrative allowances from previous years may be carried forward.

Requires utilities to remit collected Energy Assistance Charges to the Department of Revenue by the 20th day following collection, with collected funds used to assist that utility's customers and coordinate an equitable distribution methodology.

Makes Section 13 subject to repeal on December 31, 2018 unless renewed by the General Assembly, with the repeal decision to consider evaluation results from the program.
