Creates the DUI Prevention and Education Commission with 5 members from the Secretary of State, State Police, Department of Human Services, Department of Transportation, and State's Attorneys Appellate Prosecutor to review and approve DUI prevention grant applications.

Establishes the DUI Prevention and Education Fund as a special state fund to distribute grants for crash victim programs, impaired driving prevention, law enforcement support, and other DUI-related programs under Department of Transportation oversight.

Transfers $750,000 from the Roadside Memorial Fund to the DUI Prevention and Education Fund as initial funding.

Redirects $45 of a $50 supervision fee imposed on individuals convicted or placed on court supervision for violating Section 11-501 (DUI) of the Illinois Vehicle Code into the DUI Prevention and Education Fund, with $5 going to the Roadside Memorial Fund.

Allows courts to waive the $45 DUI Prevention and Education Fund fee if the defendant achieves full restitution.
